The Guana Cays IBA is located between Crooked Island and Acklins Island on the southwestern reef edge and the entrance to the Bight of Acklins. It comprises a group of small cays (including North Cay, Fish Cay, and Guana Cay) and associated rocks and reefs. The habitat for the area includes rock shoreline and shrubland.
A number of seabirds are found within this habitat, including the globally significant Sandwich Tern Sterna sandvicensis and Roseate Tern S. dougallii. Least Tern S. antillarum, Bridled Tern S. anaethetus, and Magnificent Frigatebird Fregata magnificens are also found on these Cays.

The cays can be accessed by boat from Acklins Island.
